This Privacy Policy (this “Privacy Policy”) informs you (“You” or as the context requires “Your”) what information Matthew Dowdy, MD, LLC and/or Evolution Primary Care (“EPC”) may collect, store, use, or share when You visit the health portal powered by eClinicalWorks website (the “Site”) operated by EPC. This Privacy Policy is a part of and incorporated into the Terms of Use Agreement (“Terms of Use”) applicable to the Site and any capitalized terms not defined here shall have the meanings assigned in the Terms of Use.
